Infant fatalities have occurred in inclined sleepers after the infants rolled from their back to their stomach or side while unrestrained, or under other circumstances.

    Description: The recall involves infant hammock swings made of wood and cloth with adjustable height settings. The swings were sold in the following colors: ivory, taupe gray, gray, dark gray, blue, ochre, green and pink. “CaTeam” is printed on a label stitched on the hammock swings.

    Remedy: Consumers should immediately stop using the recalled baby hammock swings and contact CaTeam for instructions on how to receive a full refund and properly dispose of the product. CaTeam and Amazon are contacting all known purchasers.

    Hazard: The recalled dietary supplements contain iron which must be in child-resistant packaging as required by the Poison Prevention Packaging Act (PPPA). The packaging of the products is not child-resistant, posing a risk of poisoning if the contents are swallowed by young children.

The recalled bottles include lot numbers M0786, M0816, M0817 and M0818, which are printed on the bottom of the bottle.

    Remedy: Consumers should immediately store the recalled dietary supplements in a safe location out of reach and sight of children and contact Nationwide Pharmaceutical for information on how to dispose of the product. Consumers can also return the product to the place of purchase to receive a refund of the purchase price.

    Description: This recall involves both full-size Robo Alive Junior Baby Shark Sing & Swim bath toys and Mini Baby Shark Swimming bath toys. The recalled full-size Baby Shark bath toys sing and swim when placed in water. They have a hard plastic top fin with three grooves on one side, measure about 7-inches from nose to tail and were sold in three colors: yellow, pink and blue. They were sold individually and in packs of three. The recalled full-size bath toys have tracking label information on the bottom, including raised lettering that states model number “#25282” and a date code beginning with the letters “DG” followed by “YYYY/MM/DD” in the date range DG20190501 through DG20220619. Only full-size Baby Shark toys with a hard plastic top fin are included in this recall.

    The recalled Mini Baby Shark bath toys swim when placed in water but do not sing. They have a hard plastic top fin, measure about 4-inches from nose to tail and were sold in three colors: yellow, pink and blue. They were sold individually, in packs of two or three, and as part of a Baby Shark Music Water Park playset. The recalled mini-size bath toys have tracking label information on the bottom, including raised lettering that states model numbers “#7163,” “#7175,” “#7166,” or “#25291” and a date code beginning with the letters “DG” followed by “YYYY/MM/DD” in the date range DG2020615 through DG2023525.

    Remedy: Consumers should immediately stop using the recalled bath toys and contact Zuru for a full refund of $14 for each full-size bath toy or $6 for each mini-size bath toy, in the form of a prepaid virtual Mastercard. Consumers should disable the tail fin (by cutting it on the full-size bath toy or by bending it on the mini-size bath toy), mark the body of the shark bath toy with the word “recalled” and the unique code provided during registration for the recall, then upload a photo of the product, showing it is disabled and marked, at https://www.recallrtr.com/bathshark. Upon receipt of the photo, Zuru will issue a refund to purchasers.

    Incidents/Injuries: Zuru is aware of 12 reports of children falling or sitting onto the recalled full-size Baby Shark bath toy, resulting in impalement injuries, lacerations and puncture wounds, including to children’s genital, anorectal and facial areas. Nine of the incidents required stitches or medical attention. There have been no reported incidents or injuries involving the Mini Baby Shark bath toys.

    Sold At: Walmart, CVS Pharmacy, Dollar General Corp., Family Dollar Services, HEB Grocery Company, Meijer, Target, TJX Companies, Ross, and Walgreens stores nationwide and online at www.walmart.com, www.target.com, www.amazon.com and other websites. The full-size Baby Shark bath toys were sold from May 2019 through March 2023 for between $13 and $15. The Mini Baby Shark bath toys were sold individually and in multi-unit packs from July 2020 through June 2023 for between $6 and $20.

    Description: This recall involves the Hurtle multi-purpose children’s helmet. The helmets were sold in black, blue, pink, white and red. The helmets have the word “Renegade” on the side in white or black lettering. The straps are black and the white warning label on the inside of the helmet states “Renegade” in black and orange lettering. The helmets are sized to fit head circumferences from about seven inches to about eight inches.

    Remedy: Consumers should immediately stop using the recalled helmets and contact Sound Around to receive a full refund of the purchase price. To receive a refund consumers should destroy the helmet by cutting off the straps, upload photos of the helmet with the straps cut to the recall registration website and then dispose of the recalled helmet. Consumers who received the unit as a free promotion should dispose of the unit so that children cannot use it.

  • Child Seat Webbing May Fail/FMVSS 213 & 209

    Recall Summary

    Name of Product: CHILD SEAT

    Hazard: Baby Trend, Inc. (Baby Trend) is recalling certain Hybrid 3-in-1 Combination Booster Seat-Desert Blue child seats. The latch assembly webbing that secures the child restraint system to the lower anchorages may fray, which can reduce the strength of the webbing and fail to properly restrain a child in a crash. As such, these child seats fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 213, “Child Restraint Systems,” and 209, “Seat Belt Assemblies.”

    Remedy: Baby Trend, Inc. will mail owners a replacement latch assembly webbing and installation instructions, free of charge. Owner notification letters are expected to be mailed June 30, 2023.

The tabletop activities include a grooved track for vehicle play, a bell arch, a bead and wire maze, a shape sorter, gear activity and stacking activity. The model number J04402 or J04411 can be found on a label on the underside of the activity table.

    Company Announcement

     05/15/23– San Juan, PR, Novis PR LLC is voluntarily recalling Lot D20911 Exp 10/25 of G-Supress DX Pediatric Drops to the consumer level. Some cartons of the product have been found to contain incorrect product inside. Incorrect product inside is an anesthetic/analgesic and not a brand of Novis PR LLC.

    Anesthetic/Analgesic product contains 60% ethyl alcohol and 5% benzocaine. There is a probability of serious adverse events with a product containing alcohol including alcohol toxicity. Infants and young children are prone to profound hypoglycemia coma, and hypothermia from ingesting relatively small amounts of ethanol, and deaths have been reported. Furthermore, the product contains benzocaine but does not include a Warning for methemoglobinemia which is a condition in which too little oxygen is delivered to your cells that can be life-threatening. To date, Novis PR LLC has not receive d any reports of adverse events or injuries related to this recall.

    G-Supress DX Pediatric Drops is a cough suppressant, expectorant and nasal decongestant used for the temporarily relief of the common cold symptoms, supplied in 1 oz. bottles packaged in a carton box. Oral anesthetic/analgesic liquid used for temporary relief for the mouth and gums supplied in 0.5 fl. oz. (15 mL) bottles. Lot D20911 was distributed among pharmacies in Puerto Rico.

    Novis PR LLC is notifying its distributors and customers by email and telephone calls and arranging return of recalled lot.  Consumers/distributors/retailers that have affected lot should stop using and return to place of purchase.

    Hazard: The recalled ATVs fail to comply with the requirements of the federal mandatory ATV safety standard. The Ace-branded K125 and C125 models exceed the maximum speed limitations for vehicles intended for children aged 10 and older, posing a risk of high-speed crash. For T125 and C125 models, the parking brakes do not prevent movement of the vehicle, posing a collision hazard to consumers if the vehicle is parked on an incline. ATVs that fail to meet the mandatory safety requirements pose a risk of serious injury or death.

The vehicles were sold in various colors including orange, red, blue, pink and green and marketed for children ages 10 and older. The model name “ACE POWER K125,” “ACE POWER T125” or “ACE POWER CONQUEROR” is displayed on both sides of the vehicle. The model number is stamped into the metal plate located on the left side, under the rear body. The phrase “This ATV is subject to EGL MOTOR INC’s action plan approved by U.S. Consumer Product Safety Commission” is printed on a label located under the model number plate.

    Remedy: Consumers should immediately stop using the recalled ATVs and contact EGL Motor to schedule an appointment for a free repair from an authorized repair shop. The firm will repair reflectors, headlights, chest protector, engine stop switch, parking brake, maximum speeds, and spark arrestor on the recalled models.
