Description: This recall involves Edoche’s Cubetensils sets of children’s eating utensils. Each set consists of a spoon and a matching fork with stainless steel heads and plastic handles. The handle has three segments that rotate to create design combinations. The utensils are 5.75-inches long and were sold in seven different models/designs: alphabet, moon and stars, nature, princess, rockets, trucks and veggies. The sets come in a brown cardboard box with a clear viewing window in the front. “Edoche” is printed on the bottom of the utensils and “Stainless Steel China” is embossed on the metal heads.  The phrase “Not for children under 3” appears on the back of the product’s packaging.

Incidents/Injuries: Edoche has received one report of the utensil’s handle detaching and a baby putting it in his/her mouth. No injuries have been reported.

Remedy: Consumers should immediately take the utensils away from children and contact Edoche for a full refund.

Description: The Playtex Hip Hammock is an infant carrier designed to strap the baby against the caregiver’s body at the hip.  It is made of a soft, quilted fabric and intended for babies that are from 15 to 35 pounds.  The child seat is attached with straps that wrap around the carrier’s hips and shoulder. “Playtex Hip Hammock” is printed on a label sewn into the front of the carrier.  All model numbers are being recalled.  Model numbers 05300, 05301, 05302, 05306, 05307 and 05308 are sewn into the inside panel below the instructions for use. They come in basic and deluxe styles. The hip hammock’s fabric is suede or ultra-suede in black or navy colors on the outside, and the inside lining is black, black and white check or burgundy.

Incidents/Injuries: Playtex has received 87 reports of the buckles cracking or breaking, including two reports of injuries, where one infant required emergency room treatment.

Remedy: Consumers should stop using the carrier and contact Playtex for instructions on how to return the product for a full refund.

Name of product:  Children’s wall-mounted lamps

Hazard:  A 16-month-old child in a crib died after getting entangled in the lamp’s cord.  In a separate incident, a 15-month-old child in a crib became entangled in the lamp’s cord and nearly strangled. In both incidents, which occurred in Europe, the lamp cord was pulled into the crib by the infants, creating a strangulation hazard.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with IKEA North America, of Conshohocken, Pa., is announcing the recall for repair of children’s wall-mounted lamps due to a strangulation hazard.  A 16-month-old child in a crib died after getting entangled in the lamp’s cord.  In a separate incident, a 15-month-old child in a crib became entangled in the lamp’s cord and nearly strangled. In both incidents, which occurred in Europe, the lamp cord was pulled into the crib by the infants.  There were 2.9 million of the recalled lamps sold in the United States. In addition, 1.1 million were sold in Canada. There was a total of 23 million sold worldwide.

The recalled IKEA children’s SMILA-series wall-mounted lamps were sold in eight designs, including a blue star, yellow moon, pink flower, white flower, red heart, green bug, blue seashell and an orange seahorse.

The blue star is the STJÄRNA model with article numbers 501.944.49 or 500.108.79. The yellow moon is the MÅNE model with article numbers 701.944.48 or 700.108.40. The pink flower is the BLOMMA model with article numbers 901.944.47 or 000.979.50. The white flower is the BLOMMA model with article number 300-746-50.  The red heart is the HJÄRTA model with article numbers 202.256.59 or 801.993.13. The green bug is the BAGGE model with article numbers 101.944.46 or 700.728.71.  The blue seashell is the SNÄCKA model with article number 400-982-50. The orange seahorse is the SJÖHÄST model with article number 900-982-43.

The model name is printed on a label on the inside back of the lamp near the light bulb. The article number is printed on the lamp’s packaging.

The plastic wall-mounted children’s lamps measure about 11 inches high by 11 inches wide. They have a 7 ft. long electrical cord with an in-line switch and take a 25-watt light bulb.

They were sold exclusively at IKEA stores nationwide, in IKEA’s catalog and online at ikea-usa.com from July 1999 through May 2013 for between $10 and $13.

Consumers should immediately stop using the recalled lamp and contact IKEA for a free repair kit.  Do not use the lamp until the repair kit is installed.  The repair kit has self-adhesive fasteners for attaching the lamp’s cord to the wall as well as safety instructions. Name of Product: IQ Girls’ Hooded Pink Leopard Jackets

Hazard: The jackets have a drawstring through the hood which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines about drawstrings in children’s upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC’s actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.

The jackets have pink and black leopard print drawstrings and pink and black leopard print on the sides of the jacket. “IQ girls” is printed on the sewn-in label located on the back of the neck. The model number included in the recall is 86557 and can be found printed on a sewn in label in the back of the neck. This recall involves jacket sizes girls 4 through 12.

Description: This recall involves Solowave Tornado brand home playground tube slides with port holes. The slides are green, six or seven feet long and have three or five port hole-style plastic windows in the sides. The slides were sold with Solowave’s Centennial, Centennial II, Lexington, Rocky Mountain Deluxe and Sheridan play systems and were also sold separately.  Solowave Design is stamped on the outside of the slides near the middle of each tube slide section.

Incidents/Injuries: Solowave has received 23 reports of minor injuries to children, including cuts and scrapes, from contact with broken port holes.

Remedy: Consumers should stop using the recalled slide immediately and contact Solowave to receive a free repair kit for the port hole windows.

Description:

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Angelcare Monitors Inc.®, of Quebec, Canada, is announcing a voluntary recall to provide cord covers for 600,000 Angelcare Movement and Sound Monitors with Sensor Pads. The cord attached to the baby monitor’s sensor pad is placed under the crib mattress, which poses a strangulation risk if the child pulls the cord into the crib and it becomes wrapped around the neck.

Angelcare and CPSC have received reports of two infant cord strangulation deaths. In November 2011, a 13-month-old female died in San Diego, California, and, in August 2004, an 8-month-old female died in Salem, Oregon.  In both fatalities, the cord from the sensor pads was pulled into the crib by the infant. In addition, there have been two reports of infants who became entangled in cords of Angelcare baby monitor models, which did not result in fatalities. In these incidents, it could not be determined if the “sensor pad cord” or the “monitor cord” was involved in the incident.

The recall involves the Movement and Sound Monitor manufactured by Angelcare. This design of baby monitor includes a unique sensor pad placed inside the crib, under the mattress, to monitor movement of the baby.  An electrical cord about 11 feet long is permanently connected from the sensor pad to the nursery monitor unit. The hazard is created by a cord within reach of a baby inside the crib. The cord can be pulled into the crib and can wrap around the child’s neck. The recall involves all versions of Angelcare sensor monitors including model numbers:   AC1100, AC201, AC300, AC401 AC601 and 49255 that did not include rigid cord covers, offered in the remedy.  The model number is located on the back of the nursery monitor unit. The monitors were manufactured between 1999 and 2013.

Angelcare is providing consumers with a repair kit that includes rigid protective cord covers through which the sensor pad cords can be threaded, a new, permanent electric cord warning label about the strangulation risk, and revised instructions.

The recalled baby monitors were sold at Babies R Us/Toys R Us, Burlington Coat Factory, Meijer, Sears, Walmart, Amazon.com, Target.com, Overstock.com and nearly 70 small baby specialty stores, from October 1999 through September 2013 for about $100 to $300.

Consumers should immediately make sure cords are placed out of reach of the child and contact Angelcare toll-free at (855)355-2643 between 8 a.m. and 8 p.m. ET Monday through Friday or visit the firm’s website at www.angelcare.com to order the free repair kit.

CPSC Safety Alert Campaign on Monitor Cords

In February 2011, CPSC issued a safety alert warning consumers that industry-wide there had been seven reports of strangulation in baby monitor cords since 2002.  Since that alert, the number of death reports has risen to eight of which two involved the Angelcare monitors with sensor cords. CPSC has a safety alert Infants Can Strangle in Baby Monitor Cords (link to latest version 022311) and conducted an information and education campaign with JPMA in which Angelcare has taken an active role to raise awareness on the hazards associated with baby monitor cords.  Parents and caregivers should visit CPSC’s Crib Information Center at www.cpsc.gov/cribs for additional baby monitor cord safety information and they should make sure all cords are out of arm’s reach of children.

Description: The ElliptiGO 3C, 8C and 11R models are outdoor elliptical cycles used by adults for exercise. It is a scooter-like device that combines an elliptical trainer with a bicycle. The ElliptiGO cycles have an aluminum frame, drive arms, two 20” spoked-wheels, an internally geared hub, front and rear brakes and adjustable-height steering column. All cycles have the word “ElliptiGO” along the outside of the frame.  The 3C and 8C model cycles have a “C-Series” decal towards the rear of the frame and aluminum drive arms. The recalled 11R cycles are matte black with white markings and the “11R” decal is found towards the rear of the frame.  The serial numbers on all models are located on the frame in front of the back wheel near where the kickstand attaches to the frame.  Serial number ranges for the recalled 3C and 8C models are from 11-010-001 through 13-028-102, and for the 11R model from 12-003-069 through 12-020-035.

Incidents/Injuries: ElliptiGO has received one report of the fork separating and ten reports of the drive arm detaching. One injury consisting of a minor abrasions caused by a drive arm detaching has been reported.

Remedy: Consumers should stop using recalled ElliptiGO cycles until they have the fork upgraded with a safety retrofit and/or upgraded replacement drive arm axles installed. Consumers with affected cycles can contact ElliptiGO online at www.elliptigo.com/safety to locate the authorized repair center closest to them.

Description: The recall includes Girl’s or Little Girl’s jersey knit aurora purple pajama sets sold by L.L. Bean. The sets have a solid purple top with long sleeves and purple pants with a pattern and a solid purple waistband. The pajamas were sold in girls sizes small (size 8) through extra large (size 18) and little girls sizes small (size 4) through large (6X/7). The pajamas product ID numbers included in the recall are 284889 and 284890 printed on the second side seam label under the care label. GPU#6 is also printed on the garment label located on the inside left seam near the bottom of the pajama top. L.L. Bean is printed inside the back of the neck of the garment.
