    Remedy: Refund. Consumers should immediately stop using these recalled Children’s Tool Kits and keep them away from children. Consumers should contact Grizzly Industrial for instructions on returning the recalled tool kits to receive a full refund including return shipping.     Description: The recalled Children’s Tool Kits have two different model numbers: H3044 and H5855. Model H3044 is a 12-piece tool kit that contains a helmet, goggles, tape measure, tool belt, hammer, pair of leather gloves, ruler, carpenter’s square, level, bottle of chalk and suspenders. Model H5855 is an 11-piece tool kit that contains goggles, tape measure, wooden tool caddy, hammer, ruler, carpenter’s square, level, screwdriver, pliers and an adjustable wrench. Model numbers are printed on the front upper right hand corner of the packaging. “Grizzly Industrial” is printed on multiple items of the tool kits.

  • Belle Chemical Recalls Sodium Hydroxide and Potassium Hydroxide Products Due to Failure to Meet Child-Resistant Packaging Requirement and Violation of FHSA Labeling Requirement

    Name of product: Sodium Hydroxide, Lye/Caustic Soda and Potassium Hydroxide, Caustic Potash Ash

    Hazard: The products contain sodium hydroxide or potassium hydroxide which must be in child resistant packaging as required by the Poison Prevention Packaging Act (PPPA). The packaging of the products is not child resistant, posing a risk of chemical burns and irritation to the skin and eyes. In addition, the label on the product violates the Federal Hazardous Substance Act (FHSA) by omitting the word “poison” for poisonous chemicals and the mandatory hazard statement on the front on the packaging.

    Remedy: Replace. Consumers should immediately store the recalled products in a safe location out of reach of children and contact Belle Chemical for a free replacement child-resistant packaging and label to put on the product.

  • Novartis Recalls 100 mg Sandimmune and Neoral Prescription Drug Blister Packages Due to Failure to Meet Child-Resistant Packaging Requirement; Risk of Poisoning

    Name of product: Sandimmune® (cyclosporine capsules, USP) 100 mg soft gelatin capsules and Neoral® (cyclosporine capsules, USP) MODIFIED 100 mg soft gelatin capsules prescription drug blister packages

    Hazard: The prescription drug packaging is not child resistant as required by the Poison Prevention Packaging Act (PPPA), posing a risk of poisoning if the contents are swallowed by young children.

    Remedy: New Instructions, Repair. Consumers should immediately secure the product out of the sight and reach of children and contact the firm to request a free child-resistant pouch in which to store the blister package medications. Consumers can continue to use the medication as directed. The child-resistant pouches should be used to store these medications until new child-resistant blister packaging is available.

The recalled blister packages have “Novartis,” the name of the medication, dosage, NDC, lot number and expiration date on the outer package and on the blister cards.     Incidents/Injuries: CPSC has received two reports of chemical burn injuries with the California Soap Supply product. One involved a child who received chemical burns to their mouth after gaining access to the product.

    Description: This recall involves Step2’s Little Helper’s Shopping Cart with the following model numbers and color combinations: 700000 (blue basket with tan cart,) 708500 (pink basket with white cart) and 8567KL (pink basket with gray cart). The grocery shopping carts measure 18.5 inches by 13 inches by 21.5 inches and have a shopping cart base and basket, four wheels and a doll seat. Only units with date code combinations of 5 and 19 are included in this recall. The date code can be found on the center rib on the back of the basket below the handle. The model number is located on the product packaging.

    Incidents/Injuries: Step2 has received 22 reports of cart baskets breaking. No injuries have been reported.

  • Kolcraft Recalls Inclined Sleeper Accessory Included with Cuddle ‘n Care and Preferred Position 2-in-1 Bassinets & Incline Sleepers to Prevent Risk of Suffocation

    Name of product: Inclined sleeper accessory included with Kolcraft Cuddle ‘n Care 2-in-1 Bassinet & Incline Sleepers and Preferred Position 2-in-1 Bassinet & Incline Sleepers.

    Hazard: Infant fatalities have been reported with other manufacturers’ inclined sleep products, after the infants rolled from their back to their stomach or side, or under other circumstances.

    Remedy: Refund. Consumers should immediately stop using the recalled inclined sleeper accessory and contact Kolcraft for a $35 voucher to be used on www.Kolcraft.com or a $20 refund. The voucher can be used until February 20, 2022. Kolcraft is contacting all registered owners and known purchasers directly via a postcard by mail.     Description: This recall involves all model year 2018 and 2019 Teammachine SLR01 DISC bicycles and framesets. The product is a performance road bike. BMC is printed on the downtube. The bikes and framesets were sold in red, green, grey, graphite, blue and aqua green. Recalled models contain serial numbers starting with 8R17 and 8R18, followed by one letter and a four-digit code, which is printed on the underside of the bottom bracket of the bicycle. Only bicycles and framesets with a FK6 fork require a replacement fork, which will be confirmed by BMC authorized dealers during the free inspection.

    Incidents/Injuries: BMC has received four reports of the fork steerer tube breaking while in use, with one consumer experiencing a shoulder injury that required hospitalization and surgery, and the other consumer reporting minor scratches and bruising.

  • BCI Burke Recalls Merge Playground Climbers Due to Entrapment Hazard

    Hazard: A welded rung opening on the sides of the climber poses an entrapment hazard to children.
