Description: This recall involves Little Wishes Chenille Stuffed Rattles, including the Pink & Green Fish rattles, item number 5141577, and the Blue & Yellow Fish rattles, item number 5127642. The rattles are made of a soft chenille fabric with a fiber stuffing. They are 8.5 inches by 7 inches and have a hole cut out in the middle. The item number is printed on the top left corner of the product hang tag. Rattles have a sewn-in label with “Reg. No. PA-15130(CN)” and “Hobby Lobby 9123069” printed on the front of the label.

Description: The Twin Go Original Carrier is an infant carrier designed to carry one or two babies against the caregiver’s body at the waist. It is intended for babies that are from 10 to 45 pounds. “Twin Go” is printed on a label located on the front of the carrier and an 11-digit batch number is printed on a label sewn to the waistband. Batch numbers included in this recall: 23014024003, 01515026003, and 21615019001. They were sold in fabric that is black on the outside and blue or orange on the inside.

Incidents/Injuries: The firm has received 13 reports of buckles breaking worldwide, including 10 in the U.S., one in Canada and two in the UK. No injuries have been reported.

Description: This recall involves Munchkin’s Latch lightweight pacifiers and clips sold as a set. The pacifiers were sold in five styles: designer, rattle and heartbeat clips with 0m+ natural shape pacifiers, and designer and rattle clips with 6m+ orthodontic pacifiers. The designer pacifiers and clips 0m+ and 6m+ are in three color patterns: blue and white strips, orange and with white polka dots and pink with white polka dots. The rattle pacifiers and clips 0m+ and 6m+ are green with beads in the pacifier cover to make a rattle sound and have a polka dot strap. The heartbeat pacifiers and clips have a red, heart-shaped pacifier cover and red and white polka dots on the strap.

Incidents/Injuries: The firm has received 10 reports (5 in the U.S. and 5 in Canada) of the clip cover detaching from the pacifier clip. No injuries have been reported.

Description: This recall involves Moogy plush fastening toys for toddlers between 12 and 36 months of age. The toy has zippers, buttons, buckles and laces. The Moogy toy has a blue and green face, red ears, a blue jacket with a red zipper, pink/red striped pants and pink and orange shoes with polka dots. Moogy measures about 18 ½ inches tall. “Miniland,” item number R.96295 and lot number 0115 1402813 085 are printed on a white tag sewn into the toy’s pants.

Incidents/Injuries: Miniland has received one report of the button on the pocket falling off when a baby was playing with the toy.

Description: This recall includes three models of the Fisher-Price cradle swings: CHM84 Soothing Savanna Cradle ‘n Swing, CMR40 Sweet Surroundings Cradle ‘n Swing, and CMR43 Sweet Surroundings Butterfly Friends Cradle ‘n Swing. The swings have two different swinging motions – rocking side-to-side, or swinging head-to-toe, and six different swing speeds from low to high. Incidents/Injuries: Fisher-Price has received two reports of a seat peg coming out from the seat, causing the seat to fall. No injuries have been reported.

Announcement:

Naples, FL – Back to Nature Foods, LLC is voluntarily recalling two (2) lots of its Classic Crème cookies, 12 oz packages, because it may contain undeclared milk, not listed as an ingredient on the label. Persons who have an allergy or severe sensitivity to milk run the risk of a serious or life-threatening allergic reaction if they consume this product.

The affected product was distributed to retail stores nationwide. Packages are marked with a unit UPC #19898-01103 and with best by dates of 10 SEP 16 and 16 SEP 16 labeled on top of the package. No other best by dates are affected.

Consumers who have purchased the Back to Nature Classic Crème cookies with the UPC and dates noted, and have an allergy to milk, should destroy the product they have or are urged to return to the place of purchase for product replacement or refund.

Consumers with questions may call Back to Nature’s Consumer Relations Center at 844-275-5845. The center is open Monday through Friday from 9:00 a.m. to 5:00 p.m. Eastern. Consumers also may contact the center via e-mail by visiting the Contact Us page at www.backtonaturefoods.com for a replacement coupon.

Description: This recall involves only the yellow plastic trapeze rings. They are triangular in shape with rounded sides and have a loop at the top. They measure about 8½ inches high by 6½ inches wide. The yellow rings come as a pair and were connected to a trapeze bar. They were sold either as a separate component or as an attachment on the following Rainbow®-branded residential wooden playsets: All-American, Backyard Circus, Carnival, Fiesta, King Kong, Monster, Sunray, Sunshine and Rainbow. All of these playsets have an aluminum plate located on the front of the wooden swing beam with the following name stamped on it, “Playgrounds America,” “Rainbow Play Systems Inc.,” or “Sunray Premium Playgrounds.”

Incidents/Injuries: Rainbow has received more than 100 reports of the rings cracking or breaking, including 15 with reports of injuries consisting of bumps, bruises, lacerations, concussion and one broken finger.

Sold at: Rainbow dealers nationwide from January 2007 through December 2011 and at several mass merchandisers including Sam’s Club, Toys R Us and Walmart from January 2009 through December 2009. The playsets retailed for between $900 and $10,000.

Synopsis:

Evenflo Company, Inc. (Evenflo) is recalling certain Transitions 3-in-1 Combination Booster Seats, model numbers 34411686, 34411695, and 34411029, produced from December 18, 2014, through January 29, 2016. The central front adjuster (CFA) button that is used to loosen the seat’s internal harness may be within the child’s reach, allowing the child to activate the CFA and loosen the internal harness. If the internal harness is not tightened snugly around the child, the child would be at an increased risk of injury in the event of a crash. Evenflo will notify owners and provide a remedy kit that includes a newly-designed seat pad and CFA assembly, free of charge. The recall is expected to begin during February 2016. Note: This safety issue only impacts the use of the seat in the forward-facing harnessed booster (22-65 lbs. and 28-50 in.) configuration.
